Event Participation Waiver & Photo Release


Code of Conduct 
I understand and agree that I am willingly participating in a Friends of Herring Run Parks volunteer activity and that the representative(s) of Friends of Herring Run Parks has (have) the sole authority to make decisions regarding my continued participation in the volunteer event if they determine that my conduct, for whatever reason, poses an immediate risk of harm to me or to others.
 
Photo Release
I give permission for any photos or audio recordings taken of me during this program to be made visible by Friends of Herring Run Parks on its website, social media or other published materials. 

Waiver of Liability 
I understand that participation in any Friends of Herring Run Parks event is voluntary. Therefore, any loss or injury suffered by me or to my property because of my participation in any activity or use of equipment or facilities during this event, whether sponsored or unsponsored, formal or informal, or shared or alone, is my sole responsibility. Friends of Herring Run Parks, Inc., its Board of Trustees, volunteer event leader(s), volunteer event coordinator(s), and all representatives will not be held liable for any such loss or damage to my person or property. 

Medical Emergency
In the event of an emergency and I am unable to act on my own behalf, I authorize the representative(s) of the Friends of Herring Run Parks, Inc. to take whatever action they deem is warranted and appropriate regarding my health and safety. I will not hold the representative(s) liable for their actions to assist me. 

I hereby certify that I am 18 years of age or older and have read and understand the above paragraphs before signing/agreeing to these terms.
